We have special pricing for corporate membership plans for those employers who want to offer preventive services to their employees within a reasonable cost structure. The providers at Personal Medicine of KY currently maintain contracts with the insurance plans in the Purchase Health Partners system for our traditional office based services. Be sure to be knowledgable in your plan and maintain current payments on your co-pays, co -insurance and deductibles at time of service. We'll ask that you bring a copy of your insurance card to each of your visits and update us before each change in your plan. All co-pays, co insurance and deductibles are due at time of service with no exceptions.
